Schedule shows number of full work days we require for each
case.
In scheduling
cases, do not count Saturdays, Sundays or holidays, as working days.
Show date of mailing and also date desired in your office on the
prescription form. Allow for travel time also.

Time wanted — check time schedule.
3.
Is
shade marked — for ceramic or acrylic veneer send shade tab?
4.
What shade guide was used?
5.
Did doctor
select mold or is mold optional?
6.
Age
and sex of patient?
7.
Is
bite enclosed? Opposing cast?
8.
Is
case for try-in or finish?
9.
Is
there pre-extraction or study cast to aid us?
10.
If
partial — full cast or acrylic saddles?
11.
Does doctor want spaces or irregularities — retained reduced or
eliminated?
12.
Use
separate prescription for each case — including back from die trim
and adjustments.
13.
Patient name should be clearly printed on each prescription. |
